Terms and Conditions:
1) This program is available to all Authorized Partners (Gold, Silver, Bronze, & National Accounts).
2) Valid on all orders received on or after April 1, 2015 and available for immediate shipment – no delayed ship dates.
3) Payment will be made approximately 30 days after SPIFF form is submitted.
a) A SPIFF form must be completed for each month’s sales. The form can be found on the Partner Resources Site.
iii) Each sales person at the Authorized Partner must complete and return a W9.
iv) Checks will be mailed directly to the address found on the W9.
v) 1099 will be sent at the end of the year.
